Can you remove two of these from their VLAN put them into two different new VLANs and connect the ports together. This can then verify that you get the normal link up for the modules. If this works it will verify the basic optics and the fiber.
Next test would be add a copper port to each of these VLANs (all just layer 2). then connect devices to ping across the link. You should see the traffic on the ports and be able to reach the devices.
If all this works then the EX2200 side is good and you need to concentrate on the connecting device. If any of these don't work you have issues with the port, SFP or fiber here.
