MAC-Based Authentication EX4300 - AD Radius Server

Hi all, 

I'm trying to configure MAC-Based authentication on my Juniper EX4300 switch leveraging Windows Domain Controller as the Radius Server and an object in Active Directory which contains the MAC address of the end device.

1. I have created the Radius Client on Windows Server, and created the Connection Request Policy and Network Policies following guides I found online.
2. I created an AD user for the end device using MAC address as username and password.
3. On the Juniper, I have added the Radius Server information in the Security > 802.1x panel and then applied the configuration to a test interface.

The issue I'm having is that instead of using the information in active directory for the object I'm getting a pop-up on the system when i connect it to the port asking for 802.1x Account Name and Password (screenshot).

This is the first time I'm attempting something like this and I am by no means an expert so any guidance would be appreciated. Let me know if any additional information would be helpful.
